OASIS-E Spotlight: Recall

July 24, 2023

Making sense of CMS guidelines is more challenging than ever after the launch of OASIS-E and the abundance of changes that go along with it. KanTime’s OASIS Spotlight series will spotlight specific OASIS items to simplify the ruling and help clinicians significantly improve their understanding of OASIS so they can move forward with accuracy.

C0400: RECALL

This week we’ll continue with the items that make up the BIMS.

Time points:

    • Start of Care
    • Resumption of Care
    • Discharge from agency

Recall

Ask patient: “Let’s go back to an earlier question. What were those three words I asked you to repeat?”

If unable to remember a word, give cue (something to wear, a color, a piece of furniture) for that word.

    1. Able to recall “sock”
Code 0, No – could not recall
Code 1, Yes, after cueing (“something to wear”)
Code 2, Yes, no cue required
    1. Able to recall “blue”
Code 0, No – could not recall
Code 1, Yes, after cueing (“a color”)
Code 2, Yes, no cue required
    1. Able to recall “bed”
Code 0, No – could not recall
Code 1, Yes, after cueing (“a piece of furniture”)
Code 2, Yes, no cue required
    • Allow up to 5 seconds for spontaneous recall of each word.
    • For any word that is not correctly recalled after 5 seconds, provide the category cue used in C0200.  Category cues should be used only after the patient is unable to recall one or more of the three words.
    • Allow up to 5 seconds after category cueing for each missed word to be recalled.
    • If on the first try (without cueing), the patient names multiple items in a category, one of which is correct, they should be coded as correct for that item.
    • If, however, the assessing clinician gives the patient the cue and the patient then names multiple items in that category, the item is coded as could not recall, even if the correct item was in the list.

Stay tuned for next Monday’s spotlight so your practice can continue to stay ahead of the curve and provide elite patient care through the Oasis E changes.

We provide clinicians that are part of the Kantime enterprise system with links to the guidance from CMS for each individual OASIS item.
