M0100, This Assessment is Currently Being Completed for the Following Reason =
9. Discharge from Agency
– AND –
M2420, Discharge Disposition =
1. Patient remained in the community (without formal assistive services)
– OR –
4. Unknown, because patient moved to a geographic location not served by this agency
– OR –
UK. Other unknown
Indicate the route(s) of transmission of the current reconciled medication list to patient, family, and/or caregiver.
