This item is intended to identify the patient’s self reported ethnicity data.

Are you of Hispanic, Latino/a or Spanish origin?
Code A, No, not of Hispanic, Latino/a, or Spanish origin
Code B, Yes, Mexican, Mexican American, Chicano/a
Code C, Yes, Puerto Rican
Code D, Yes, Cuban
Code E, Yes, another HIspanic, Latino, or Spanish origins
Code X, Patient unable to respond
Although the intent is for the source of information on this item is patient reported ethnicity, CMS will allow the information to be obtained from either a proxy or the medical record.

- If a patient is unable to respond:
- A proxy response may be used.
- If neither the patient or a proxy is able to provide the information, medical record data may be used.
- If a proxy and/or medical record data are used, check all boxes that apply INCLUDING Code X, patient unable to respond.
- If a patient is unable to respond:
Code Y, Patient declines to respond
- If the patient DECLINES to respond, do not use information obtained from a proxy or the medical record.
